New Minor Passport Book Information

What you need to apply:

 

All minors regardless of age, including newborns and infants, must have their own passport when traveling internationally by air. U.S. citizen children under age 16 arriving by land or sea from Mexico, Canada, and the Caribbean may present an original or copy of his or her birth certificate, a Consular Report of Birth Abroad, or a Naturalization Certificate.

What you need to apply:

- A completed DS-11 Form  (also can be picked up at the Clerk's Office)

- Evidence of child’s U.S. citizenship (Original Birth Certificate)

- Parent(s) or guardian(s) identification and photocopy of identification

The child and both parents must be present. If a parent cannot be present, please provide the second parents' notarized Statement of Consent (Form DS-3053), along with a copy of the front and back of the parent's identification or driver's license.  If the second parent is deceased, please bring an original death certificate.

- One 2”x 2” passport photo

- Proper Payment of Passport Fee
